Graduated from Wabash College with an A.B. in Biology, then attended Indiana University School of Medicine receiving an M.D. in 1984. He completed a Transitional (Flexible) Residency at Methodist Hospital of Indiana in 1985, then moving on to Indiana University Medical Center completing residency in Anesthesiology in 1987. He obtained board certification from the American Board of Anesthesiology in 1989.

Currently he is the Director of the Transitional Residency at Ball Memorial Hospital in addition to holding the rank of Associate Clinical Professor of Anesthesia at Indiana University. Has been a member of CTYPD (Council for Transitional Year Program Director) and AHME since 1992. Has served as an at-large member of the Board of CTYPD since 1994 and also on the Member Services Committee since 2002. He served as Chair for the CTYPD from
1998-2000. Dr. Costello has been a member of the Indiana Society of Anesthesiologists since 1989, serving past terms as Secretary/Treasurer (1994-1995), President-elect (1996-1997), and President (1998). Dr. Costello has been a member of the American Society of Anesthesiology since 1987 currently serving as the District Director and a member of the Residents and Medical Students Committee.

In the past he has served on numerous reference committees and been a delegate to the annual meeting. Dr Costello’s local involvement has included being the Secretary/Treasurer (1988-1989) and President (1991-1992) of the Delaware County Medical Society. He currently serves on the Delaware County Board of Health. Dr. Costello is married with three daughters. His outside interests include family activities, outdoor sports, and magic.
